Short-term Rehabilitation

For residents who will be returning home or to an assisted living center we offer:

• Home evaluations – we assess the resident’s home for safety issues and make any recommendations needed
• Adaptive/medical equipment – we order any equipment needed and covered by Medicare (i.e. walkers, commodes, canes)
• Home exercise programs – our therapists provide specialized home exercise programs
• Assistance in setting up services (i.e. home care, meals, etc.) when returning home
